Participant Management

ABA Care will work with each participant to develop an individualised care and service plan that meets their needs and aspirations. ABA Care recognises that each participant is unique, and each care plan is specifically tailored to the needs of the participant.

Assist Life Stage

We believe that having a choice and control over one’s life is of paramount important.  We understand transitioning through life stages can be very overwhelming and out support team will help you connect with the support that supports that suits your individual needs. We  provide support for all levels to make the transition easy and smooth. The support we provide includes:

a) Support connections-Establishing support networks

b) Plans of support -short & long term

b) Resolving points of crisis
c) Assistance applying with accommodation and rental tenancy needs
d) Reading & understanding utility bills
e) Assistance with decision making including a budget plan-Budgeting

Assist Personal Activity

Assistance for participants with a disability living alone in their own home, living with family or other people to make their life easy and comfortable when undertaking social, recreational, educational or employment activities or during holidays away from home with assistance and supervision of personal tasks of daily life. We assist with:

a) Personal hygiene, including showering, bathing, oral hygiene, dressing and grooming;
b) Toileting, bladder and bowel management
c) Attending appointments;
d) Use of aids and appliances, hearing and communication devices;
e) Mobility and transferring, for example moving in and out of bed and on or off the toilet;• Use of aids and appliances, hearing and communication devices;
f) Application of splints, and basic first aid due to injuries sustained as a result of a participant’s disability. 

Assistance With Transport/Travel

We will provide our NDIS clients who are unable to use the public transport system because of their disability, with Travel/Transport Support Services.

(a) Taxis and specialist transport arrangements will be organised with/for you, and they will drive you from one place to another and take you back home when you are done.

(b) ABA CARE support worker will accompany you while making use of Taxi and specialist transport services.

(c) Our NDIS staff will provide you with safe and convenient transport services to help you get to work, school, weekly trips to shopping centres for groceries and other retail shops, regular doctors’ appointments, allied health professionals, or social outing appointments. You can book our assist travel/transport staff ahead of your weekly trips or regular daily trips.

Development of Daily Living and Life Skills [Development-Life Skills]

We will focus on training our clients to develop skills that can increase their ability to live independently as well as participate in community activities. These skills range from basic activities such as locating amenities including public transportation within the community to more complex undertaking such as personal safety as well as problem-solving. These services are tailored to improve participants ability to managing challenging situations. We will provide these services in line with the participants’ care plan, across a range of settings including community access or homes.

Services include but not limited to:

(a) Health Eating-Shopping & Preparing Healthy Meals

(b) Personal Hygiene-presentation and personal grooming

(c) Physical Well-being-exercise and relaxation

(d) Money Handling Skills-Financial literacy

(e) Bill Payments

(f) Job Readiness-resume and interview preparation

(g) Undertaking Household chores

(h) Using Public Transport

(i) Communication, Interpersonal and Social Skills

(j) Dealing with Public Trustee

(k) Time Management

(l) Booking and Managing appointments

(m) Self-Care Strategies

(n) Problem solving Skills

Household tasks

We assist participants who may not be able to do household chores alone and may require extra supervision, such as: 

a) Garden maintenance 
b) Cleaning, drying, washing and vacuuming, Laundry
c) Grocery shopping 
d) Preparing and meal planning

(e) Dishwashing

(f) Ironing

(g) House maintenance such as window cleaning

(h) Any general household chores that a participant finds challenging. 

Participation in community, social and civic activities [Participate Community]

Community Participation is an NDIS funded support category that you may find in your NDIS Plan under core supports. The funding covers the support NDIS participants need to meet goals relating to participating or accessing community as well as recreational activities. At ABA Care we can arrange social, community, and recreational activities that participants can participate and engage in. We believe that encouraging our clients to partake in community activities, can improve their mental and physical wellbeing, as well as increase self-fulfillment, communication skills, and confidence.  We can support you for a few hours to a day. Activities include but not limited to:

  1. Participating in social group
  2. Attending personal development courses
  3. Going on Holiday camp
  4. Planning public transport journeys
  5. Going out to community events
  6. Joining a sporting club
  7. Going out for a movie night or attending a concert
  8. Visiting your local library

Daily Tasks/Shared Living

Our Assistance with Daily Life Tasks service supports NDIS participants living in group or shared accommodation settings.

We assist individuals with various daily activities, ensuring they can lead fulfilling and independent lives within their chosen living arrangements. This service enables NDIS participants to maintain independence while receiving the necessary support to manage daily tasks effectively. It promotes social inclusion and boosts community among residents, enhancing overall well-being and quality of life.
